Merrimack College Athletics

 All student-athletes are required to submit all medical documentation by July 1 annually, prior to the start of the academic year.  All medical documentation must be submitted through our electronic health and medical records system, SportWare.  Student-athletes will not be medically cleared until all required documentation is submitted and approved by the athletic training staff.

NO paperwork will be accepted via mail, email or fax. Please appropriately upload all documentation to the  SportsWare website utilizing the instructions above.
